Key Responsibilities
• Provide speech-language pathology services to patients in a skilled nursing facility.
• Assess, diagnose, and develop treatment plans for individuals with communication and swallowing disorders.
• Collaborate with healthcare team members to ensure comprehensive patient care.
• Document patient progress and maintain accurate clinical records.
• Utilize Rehab Optima (Net Health) software to document therapy sessions and update patient records.
Required Qualifications
• Valid California Speech-Language Pathologist license.
• Minimum of 1 year of SNF (Skilled Nursing Facility) experience.
• Strong knowledge of Rehab Optima (Net Health) documentation system.
• Excellent communication and interpersonal skills.
Preferred Experience / Skills
• Experience working in a long-term care or skilled nursing environment.
• Ability to manage a diverse caseload effectively.
