Key Responsibilities:
• Provide high-quality nursing care to residents in a long-term care setting
• Develop and implement individualized care plans to meet the unique needs of residents
• Monitor and report changes in resident condition to healthcare team
• Educate residents and their families on health and wellness topics
• Collaborate with interdisciplinary team to coordinate resident care
Required Qualifications:
• Current licensure as an LVN/LPN in the state of IA
• Basic Life Support (BLS) certification
• Ability to lift and transfer residents as needed
Preferred Experience / Skills:
• Experience working in LTC/SNF settings
• Knowledge of gerontological nursing principles
• Ability to communicate effectively with residents, families, and healthcare team
