Key Responsibilities

• Provide social work support to patients and families in their home environments across Montrose County.
• Collaborate with an interdisciplinary team including nurses, physicians, chaplains, and certified nursing assistants to deliver comprehensive care.
• Assess patient needs, provide education, resources, and advocacy to improve quality of life.
• Document patient interactions and coordinate care plans in accordance with company policies.
• Travel to patients' homes and assist in navigating healthcare and community resources.

Required Qualifications

• Minimum of two years of healthcare experience as a social worker or therapist.
• Excellent interpersonal and communication skills.
• Experience working with elderly populations preferred.
• Valid social work or therapy licensure as required.

Preferred Experience / Skills

• Prior volunteer or paid experience working with seniors or in home health settings.
• Strong ability to work independently and during travel within rural communities.
