Key Responsibilities
- Provide skilled nursing care to patients in a home health setting within East Portland, OR, and surrounding areas.
- Conduct patient assessments, develop care plans, and monitor patient progress.
- Maintain accurate charting using electronic health records (Epic, Rover, Oasis).
- Educate patients and family members on health management and care procedures.
- Travel to patient homes within designated radius, ensuring minimal drive time and efficient visit scheduling.
- Comply with all facility and healthcare regulations, including vaccination and certification requirements.
Required Qualifications
- Valid Oregon and Washington RN licensure.
- At least 1 year of experience in skilled home health nursing.
- BLS certification.
- Own transportation to travel between patient sites.
Preferred Experience / Skills
- Experience with Oasis documentation and electronic charting systems.
- Ability to work independently and manage a variety of patient cases.
- Strong communication and organization skills.
