Key Responsibilities

  • Collaborate with a multidisciplinary team to support inpatient Care Coordination and Utilization Management.
  • Lead local inpatient interdisciplinary team meetings within the Critical Access Hospital (CAH) Swing Bed/Transitional Care program.
  • Review admissions to develop personalized, clinically appropriate treatment plans aligned with patient needs, preferences, and resource availability.
  • Coordinate with providers, leadership, and support staff to ensure treatment plans meet regulatory and payor criteria.
  • Conduct patient assessments, provide psycho-social support, and facilitate referrals as needed.
  • Develop and communicate discharge plans, ensuring safe and coordinated transitions of care.
  • Support peer review processes in accordance with consortium guidelines.

Required Qualifications

  • Current full, unrestricted Registered Nurse license in Alaska or another U.S. state; applicants must have applied for an Alaska license before their start date.
  • Bachelor’s degree in Nursing preferred.
  • Case Management Certification within 24 months of hire from a recognized certifying body.
  • BLS certification; ACLS and PALS preferred.
  • Clinical competency certification within 3 months of hire and every three years.
  • High school diploma or equivalent required.

Preferred Experience / Skills

  • Minimum of 5 years of clinical nursing experience.
  • At least 1 year of experience in chart review, risk management, or related quality services.
  • Knowledge of medical record review, insurance guidelines, and regulatory compliance.
  • Strong communication and organizational skills.
  • Ability to work collaboratively as part of an interdisciplinary team and build relationships with colleagues, community partners, and patients.

Physical Demands

  • Regularly required to talk, hear, stand, walk, sit, and use hands.
  • Occasionally required to climb, balance, stoop, kneel, crouch, or crawl.
  • Must be able to lift and move up to 50 lbs.

  • ACLS (American Heart Association)
  • BLS (American Heart Association)
  • Case Management Certification (Commission for Case Manager Certification)
  • Clinical Competency and High Risk Competency (SEARHC)
  • Alaska Registered Nurse License
