Key Responsibilities

  • Manage the active care of patients in collaboration with medical providers and interdisciplinary teams to develop personalized treatment plans aimed at optimizing health outcomes.
  • Coordinate multidisciplinary care conferences for high-risk or complex patients referred into case management.
  • Oversee panel acute and chronic care needs through continuous review of electronic health records (EHR) reminders and outreach to patients.
  • Facilitate referrals within the organization and to external facilities, ensuring timely follow-up and tracking of outcomes.
  • Serve as the primary clinical support for specialty clinics, including patient assessment, education, medication administration, and support during medical procedures.
  • Act as the primary case manager for obstetrics patients, managing intake, education, postpartum care, and newborn immunizations.
  • Coordinate Video Teleconferencing (VTC) visits with specialty providers and manage referral processes for exercise treadmill tests.
  • Perform nursing duties, including patient assessment, documentation, education, medication administration, and maintaining a safe, clean work environment.

Required Qualifications

  • Graduate of an accredited nursing school.
  • Current, unrestricted Registered Nurse license in Alaska or a U.S. state.
  • Basic Life Support (BLS) certification.
  • Clinical competency within 3 months of hire and every three years.
  • At least 1 year of general nursing experience, or 6 months at a SEARHC outpatient clinic, or 2 years of nursing experience with 1 year in an ambulatory setting or case management.
  • Knowledge of SEARHC services and tribal health organizations in Alaska.
  • Strong assessment, communication, and multitasking skills.
  • Ability to lift and move up to 50 pounds.
  • Physical ability to stand, walk, sit, bend, and perform necessary clinical tasks.

Preferred Experience / Skills

  • National Case Management certification.
  • Experience with specialty clinics or obstetrics case management.
  • Knowledge of evidence-based nursing practices and shared governance models.
