Schedule / Job Type
- Full-time, day shifts, 4 days per week
Key Responsibilities
- Provide skilled nursing care in an outpatient/ambulatory setting
- Assist with patient assessments and administer treatments
- Support clinical team with patient flow and documentation
- Ensure compliance with facility policies and safety protocols
- Collaborate with healthcare providers to achieve quality patient outcomes
Required Qualifications
- Valid LVN or LPN license in the state of Vermont
- At least 1 year of experience in an urgent care, clinic, or outpatient setting
- Experience with EMR systems, specifically EPIC (within the past 2 years)
- Ability to work a flexible schedule including weekends and holidays as needed
- Reliable transportation; personal or rental vehicle preferred during winter months
Preferred Experience / Skills
- Dementia-capable care certification (preferred)
- Experience working in outpatient or ambulatory care environments
