Key Responsibilities
• Provide comprehensive physical therapy services to patients in a Skilled Nursing Facility (SNF) environment.
• Assess patient needs and develop customized treatment plans.
• Implement therapy techniques to improve mobility, strength, and overall well-being.
• Document patient progress and communicate updates to the healthcare team.
• Ensure compliance with all regulatory and safety standards.
Required Qualifications
• Minimum of 1 year of professional SNF (Skilled Nursing Facility) experience.
• Valid state license to practice as a Physical Therapist.
• Strong assessment and treatment planning skills.
• Excellent communication and documentation abilities.
Preferred Experience / Skills
• Experience working with diverse patient populations.
• Familiarity with electronic medical records (EMR) systems.
