Key Responsibilities
- Provide skilled physical therapy services to home health patients in Hood River and surrounding areas.
- Assess patient needs and develop individualized treatment plans.
- Document patient progress and treatment outcomes using electronic health records.
- Coordinate care with other healthcare providers as necessary.
- Manage a caseload of up to 6 patients per day, maintaining quality care standards.
Required Qualifications
- Valid state license to practice as a Physical Therapist in Oregon and Washington.
- BCLS certification.
- Minimum of 1 year of skilled home health experience.
- Proficiency in Oasis documentation and familiarity with EPIC and Rover systems.
Preferred Experience / Skills
- Experience working in home health settings.
- Ability to manage a flexible schedule with minimal drive time between patients.
- Reliable transportation to accommodate occasional travel within a 60-mile radius.
