Schedule / Job Type
• Full-time, day shifts, Monday through Friday, open to 32-40 hours per week
Key Responsibilities
• Provide in-home nursing care to patients within the service area of Plymouth County, Iowa.
• Perform OASIS assessments and develop individualized care plans for patients.
• Manage patient care in accordance with healthcare standards and departmental protocols.
• Participate in the department’s call rotation, approximately one in four shifts.
• Document patient interactions and treatment plans within the EMR system, Meditech.
Required Qualifications
• Valid Registered Nurse (RN) license in the state of Iowa.
• Minimum of 2 years of experience performing OASIS assessments and care planning for home health patients.
• Experience with or familiarity with Meditech electronic medical records is a plus.
• Ability to work independently and as part of a team in a home health setting.
• Strong communication and organizational skills.
Preferred Experience / Skills
• Previous experience working in rural or community-based home health environments.
• Comfort with electronic documentation and clinical assessments.
