Key Responsibilities

• Provide home health nursing care to patients in a clinical setting.
• Conduct patient assessments and develop individualized care plans.
• Perform patient phone triage and in-home visits as needed.
• Participate in on-call rotation for after-hours patient support.
• Document all patient interactions accurately and timely.
• Collaborate with interdisciplinary team to ensure quality patient care.

Required Qualifications

• Active RN license with home health experience.
• Experience with Oasis documentation for RN.
• Ability to plan for credentialing process (approx. 5-10 business days).
• Must be able to travel within a 50-mile radius of Wilmington, DE.
• In-person fingerprinting and background check required.

Preferred Experience / Skills

• Prior experience in a home health setting.
• Familiarity with telephonic patient triage.
• Comfortable working independently with minimal supervision.
