• Full-time, day shifts, working 8 hours per shift, 5 days a week
Key Responsibilities
• Provide skilled physical therapy services to patients in a Skilled Nursing Facility (SNF) setting
• Develop and implement individualized treatment plans to improve patient outcomes
• Monitor and document patient progress and adjust treatments accordingly
• Collaborate with the healthcare team to ensure comprehensive patient care
• Ensure compliance with all regulatory and safety standards
Required Qualifications
• Valid state license as a Physical Therapist
• Must have at least 1 year of professional SNF experience
• Strong knowledge of physical therapy practices and procedures
Preferred Experience / Skills
• Experience working with a diverse patient population in a SNF environment
• Excellent communication and teamwork skills
