Schedule / Job Type
- Full-time, 12-hour day shifts, working 3 shifts per week
Key Responsibilities
- Provide nursing care to long-term care and skilled nursing patients
- Assist with patient assessments and develop care plans
- Administer medications and treatments as prescribed
- Document patient care and maintain accurate records using PCC electronic health records
- Collaborate with healthcare team to ensure quality patient care
- Support rehabilitation and recovery efforts within the facility
Required Qualifications
- Valid Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) license
- Experience in LTC or SNF settings preferred
- Knowledge of PCC or similar electronic medical record systems
- Ability to work independently and as part of a team
- Strong communication and documentation skills
Preferred Experience / Skills
- Previous experience in a long-term care or skilled nursing facility
- Comfort working with general long-term patient populations and rehab patients
