Key Responsibilities
- Screen, evaluate, and treat patients in their home settings, developing individualized care plans in collaboration with physicians, patients, and caregivers.
- Coordinate patient care activities with an interdisciplinary team, ensuring seamless communication and appropriate case conferences.
- Conduct ongoing assessments of patients’ clinical, functional, and emotional status, documenting evaluations, treatment goals, and objectives.
- Educate patients and caregivers on self-care techniques, medication management, diet, and health counseling based on physician orders.
- Review and recertify patient care plans periodically, advising on discharges and communicating significant changes to physicians and team members.
Required Qualifications
- Valid RN license in Vermont or New Hampshire, or possession of an unencumbered compact license.
- Graduate of an accredited nursing school.
- Minimum of 2 years of experience in home health nursing.
Preferred Experience / Skills
- Experience coordinating care in a home health setting.
- Strong communication and interdisciplinary collaboration skills.
- Ability to adapt to a diverse patient age range and medical conditions.
