Key Responsibilities

• Provide occupational therapy services to patients in a home health setting.
• Assess patient needs and develop individualized treatment plans.
• Document patient progress accurately using clinical software.
• Collaborate with healthcare teams to ensure comprehensive patient care.
• Maintain compliance with all organizational and regulatory guidelines.

Required Qualifications

• Valid VA occupational therapy license in hand or proof of compact license.
• Minimum of 2 years of recent home health occupational therapy experience.
• EPIC charting experience required, especially for acute care hospitals under Riverside Health.
• Certification in NIH and stroke education for hospital-based roles.
• Strong assessment and documentation skills.
• Ability to work independently in a home health environment.

Preferred Experience / Skills

• Supervisory experience in occupational therapy.
• Experience working with Riverside Health facilities (if applicable).
• Flexibility to float between units as needed based on facility requirements.
• Proven ability to handle a caseload efficiently within a fast-paced environment.
