Key Responsibilities
- Provide physical therapy services to a diverse patient population in a home health setting.
- Conduct comprehensive assessments and develop personalized treatment plans.
- Perform patient evaluations and oversee therapy interventions, including post-acute care and chronic condition management.
- Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
- Document patient progress and maintain accurate records using electronic medical records (EMR) systems.
- Support oversight of therapy assistants as needed.
Required Qualifications
- Valid North Carolina Physical Therapist license.
- BLS/CPR certification.
- Experience in home health therapy.
- Ability to perform OASIS assessments.
Preferred Experience / Skills
- Knowledge of the new payment model (PDGM) effective from January 2020.
- Prior experience with post-acute care, especially in geriatric populations.
- Friendly, easy-going personality suited for in-home patient visits.
