Job Title
- LVN / LPN - Long Term Care / Skilled Nursing Facility

Schedule / Job Type
- Full-time, night shifts
Key Responsibilities
- Provide nursing care to residents in a long-term care or skilled nursing facility setting
- Document patient assessments and care plans using electronic medical records (PCC)
- Collaborate with interdisciplinary team to ensure quality resident care
- Adhere to facility policies and state licensing regulations
- Work every third weekend and holiday, with shifts lasting 12 hours
Required Qualifications
- Valid, active Minnesota LVN or LPN license
- Active Minnesota SNSA registration
- Prior experience in LTC/SNF preferred
Preferred Experience / Skills
- Knowledge of electronic medical records (PCC)
- Ability to work night shifts and handle a fast-paced environment
