Key Responsibilities

  • Provide individualized nursing care in accordance with physician orders and facility policies in a skilled nursing or LTC setting.
  • Assist with patient assessments, treatment plans, and medication administration.
  • Monitor patient conditions, respond to emergencies, and document care accurately.
  • Collaborate with healthcare team members, residents, families, and facility staff to ensure high-quality care.
  • Maintain safety and infection control standards while providing compassionate, respectful care.
  • Support patient/family education and ensure proper transfer and discharge procedures.

Required Qualifications

  • Valid California LVN or LPN license.
  • Current BLS (Basic Life Support) certification.
  • Good physical health, stamina, and manual dexterity for patient care tasks.
  • Ability to communicate effectively in English, both verbally and in writing.
  • Dependable with a strong attendance record.
  • Strong assessment skills and ability to respond to emergency situations.
  • Willingness to work beyond regular hours if needed.

Preferred Experience / Skills

  • Previous experience in LTC or SNF environments.
  • Familiarity with electronic health records and Microsoft Office applications.
  • Knowledge of infection control and safety protocols in healthcare.
  • Ability to work collaboratively within a multidisciplinary team.
