Key Responsibilities
- Provide physical therapy services to patients in a skilled nursing facility.
- Develop and implement individualized treatment plans to improve patient mobility and function.
- Monitor patient progress and adjust therapies accordingly.
- Collaborate with healthcare team to ensure optimal patient care.
- Document patient assessments, treatment progress, and discharge planning.
Required Qualifications
- Valid state license as a Physical Therapist.
- Proven experience in a healthcare or skilled nursing setting preferred.
- Excellent communication and patient care skills.
- Ability to work effectively in a team environment.
- Commitment to patient safety and confidentiality.
Preferred Experience / Skills
- Prior experience working in a skilled nursing facility (SNF).
- Knowledge of post-acute rehabilitation protocols.
- Bilingual abilities are a plus.
