Key Responsibilities
- Manage actively a panel of patients, coordinating with medical providers and interdisciplinary teams to develop individualized care plans aimed at optimizing health outcomes.
- Facilitate multi-disciplinary care conferences for high-risk and complex patients, ensuring appropriate follow-up and comprehensive care delivery.
- Oversee acute and chronic patient needs, health maintenance activities, and outreach efforts based on electronic health record (EHR) reminders, providing patient education as needed.
- Coordinate and track referrals within the organization and external facilities, ensuring timely completion and providing updates to healthcare providers.
- Serve as a primary case manager for obstetric and specialty clinic patients, including postpartum follow-up and clinic scheduling.
- Support specialty clinics by managing referrals, preparing clinical spaces, assisting in procedures, and providing clinical support to providers.
- Communicate effectively across teams via videoconferencing and manage appointment coordination for specialized testing and services.
- Perform direct patient care duties, including assessments, education, administering therapies, and maintaining documentation in a fast-paced clinical environment.
- Ensure safety and infection control standards are upheld, maintaining a safe and clean working environment.
Required Qualifications
- Current, unrestricted Registered Nurse license in Alaska or another U.S. state.
- Graduate of an accredited nursing program; Bachelor’s degree preferred.
- BLS Certification from the American Heart Association.
- At least 1 year of general nursing experience, with specific experience in outpatient care or case management preferred.
- Knowledge of SEARHC and tribal health services in Alaska.
- Strong assessment, communication, and multitasking skills.
- Ability to work independently and efficiently in a fast-paced setting.
- Clinical competency required within 3 months of hire and every three years thereafter.
Preferred Experience / Skills
- 6 months’ experience specifically as a clinical nurse at an outpatient clinic or in case management.
- National Case Management Certification.
- Experience coordinating specialty clinic referrals and telehealth services.
