Key Responsibilities

• Conduct assessments to develop and implement personalized rehabilitation programs for residents
• Provide high-quality physical therapy treatments and interventions in a skilled nursing/long-term care setting
• Collaborate with an interdisciplinary care team to enhance patient outcomes
• Educate and motivate residents to take an active role in their rehabilitation journey
• Build meaningful relationships with residents and their families
• Supervise Physical Therapist Assistants (PTAs)

Required Qualifications

• Bachelor's, Master's, or Doctorate degree from an accredited Physical Therapy program
• Current Physical Therapist license in the state of Oregon
• Current CPR certification
• Strong communication skills and ability to work collaboratively within a care team
• Ability to inspire and motivate patients toward their recovery goals

Preferred Experience / Skills

• Minimum six months of experience in a long-term care or skilled nursing facility environment
• Experience with NetHealth or similar therapy documentation software
