Social Worker

Title: Social Worker (MSW) – Acute Care Hospital
Schedule: Days with rotating weekends – typically 8:00 AM – 4:30 PM (may vary 9:00–5:30 or 9:30–6:00)

About the Facility:
This is a 267-bed acute care hospital recognized for advanced stroke and cancer services, NICU, Trauma, and multiple ERs. The Social Worker will join a collaborative Case Management team dedicated to safe, timely, and compassionate patient care.

Role Summary:
The Social Worker (MSW) provides non-clinical biopsychosocial assessments, discharge planning, crisis intervention, and patient/family support across inpatient units and the emergency department. The role ensures smooth transitions of care, addresses psychosocial barriers, and facilitates referrals to appropriate community resources.

Qualifications:

• 1 year of recent Acute Care Social Work experience required

• Must have discharge planning experience

• Experience with PASSRs and ESPERs required

• Master of Social Work (MSW) required

• Candidates with only psych or community therapy experience will not be considered

• Preferred: Hospital, LTAC, Hospice, or Insurance-based Social Work background

Ideal Candidate Profile:

• Previous hospital or insurance-based social work experience (e.g., Humana or county social work)

• Self-starter with a strong sense of urgency

• Highly organized multitasker with strong critical thinking skills

• Engaging personality and ability to collaborate with care teams and patients

Compensation & Benefits:

• Salary: $24.93/hr – $33.59/hr (based on experience)

• Relocation: Case-by-case
