Therapist - Physical Therapist Job Description

Job Summary:

We are seeking a highly skilled and compassionate Physical Therapist to join our team at BJC Home Health in Glen Carbon, IL. As a Therapist - Physical Therapist, you will provide high-quality patient care, working collaboratively with our multidisciplinary team to deliver exceptional outcomes.

Responsibilities:

• Assess and treat patients with orthopedic, cardiac, neuro, pulmonary, and diabetic conditions, utilizing evidence-based practices and case management skills. • Develop and implement individualized patient-centered goals, prioritizing their needs and promoting independence. • Supervise and support Physical Therapy Assistants (PTAs) to ensure seamless delivery of care. • Communicate effectively with patients, families, and healthcare teams to ensure seamless transitions and optimal outcomes. • Manage time efficiently, maintaining a high caseload while ensuring quality and attention to detail. • Demonstrate flexibility, professionalism, and a positive attitude in a fast-paced environment. • Adhere to BJC Home Health's policies, procedures, and standards of practice.

Requirements:

• 1+ year of experience in physical therapy, with a strong foundation in assessment, treatment, and case management. • Current licensure and certification in physical therapy (e.g., BLS, CERTS). • Ability to work independently, with minimal supervision, and as part of a team. • Strong communication and interpersonal skills. • Ability to maintain confidentiality and handle sensitive patient information.

Working Conditions:

• Work is performed in a home health setting, with occasional travel to other locations. • The therapist will work 5 days a week, Monday through Friday, 8am to 4pm. • A large parking space is available at the office.
