We are seeking a compassionate and skilled Physical Therapist to join our team. As a Physical Therapist with our organization, you will have the opportunity to make a meaningful difference in the lives of our patients, helping them achieve optimal health and wellness.
Key Responsibilities:
• Conduct comprehensive evaluations of patients to identify areas of need and develop personalized treatment plans • Design and implement evidence-based physical therapy interventions to promote mobility, strength, and function • Provide one-on-one therapy sessions, as well as group therapy sessions as needed • Collaborate with interdisciplinary teams, including physicians, nurses, and occupational therapists, to ensure comprehensive care • Educate patients and families on proper exercise techniques, body mechanics, and home safety strategies • Document patient progress and outcomes in accordance with regulatory requirements
Requirements:
• Current licensure as a Physical Therapist in the state of Iowa • Bachelor's degree in Physical Therapy or related field • New graduates are welcome to apply • SNF experience preferred
Credentialing Requirements:
• State License Verification • Resume • References (3) • County Criminal Background Check (7 years for all counties lived and worked) • Federal Criminal Background Check (7 years or more) • Multi-State Criminal Search (7 years or more) • Nationwide Sex Offender Search (7 years or more) • HHS/OIG Search • GSA Search • Social Security Number Trace • 2-Step PPD or Quantiferon • Proof of Positive PPD and Chest X-Ray • SAM Search • Iowa SING Background
