We are seeking a compassionate and skilled Registered Nurse to join our long-term care and skilled nursing facility team. As a Registered Nurse - LTC/SNF, you will play a vital role in delivering high-quality patient care and contributing to the well-being of our residents.

Key Responsibilities:

• Assess, plan, implement, and evaluate patient care to promote optimal health outcomes • Develop, implement, and update individualized care plans in collaboration with interdisciplinary teams • Administer medications, treatments, and procedures as prescribed by physicians • Provide emotional support and education to patients, residents, and families • Conduct routine medical assessments, monitor vital signs, and report changes to healthcare teams • Collaborate with interdisciplinary teams to coordinate care and achieve patient goals • Maintain accurate and up-to-date patient records and reports

Requirements:

• Current licensure as a Registered Nurse in the state of [insert state] • Minimum 2 years of experience in long-term care or skilled nursing facilities • Strong knowledge of gerontological nursing principles and practices • Excellent communication, interpersonal, and problem-solving skills • Ability to work effectively in a fast-paced, dynamic environment • Current certification in basic life support (BLS) and/or advanced cardiovascular life support (ACLS) preferred
