Registered Nurse LTC/SNF
**Registered Nurse - Long Term Care/Senior Living Facilities (LTC/SNF)**

We are seeking a compassionate and skilled Registered Nurse to join our team in Long Term Care/Senior Living Facilities (LTC/SNF). As a Registered Nurse in this role, you will have the opportunity to make a positive impact on the lives of our residents, providing high-quality patient care and supporting their unique needs.
**Key Responsibilities:**
• Provide direct patient care, including assessments, planning, implementation, and evaluation of nursing interventions
• Develop, implement, and revise individualized care plans to meet the unique needs of each resident
• Collaborate with multidisciplinary teams, including physicians, therapists, and other healthcare professionals, to ensure comprehensive care
• Conduct patient assessments, including vital signs, medical history, and psychosocial evaluations
• Administer medications, treatments, and procedures as ordered by physicians
• Educate residents, families, and staff on health promotion, disease prevention, and management of chronic conditions
• Participate in quality improvement initiatives to enhance patient outcomes and resident satisfaction
• Maintain accurate and up-to-date records, including medical charts and reports
• Adhere to regulatory requirements, including HIPAA and OSHA standards
**Requirements:**
• Current licensure as a Registered Nurse (RN) in the state of [insert state]
• Minimum 2 years of experience in LTC/SNF or related field
• Strong knowledge of nursing theories, principles, and practices
• Excellent communication, interpersonal, and teamwork skills
• Ability to work effectively in a fast-paced environment with multiple priorities
• Current CPR/BLS certification
