At Visiting Nurse and Hospice for Vermont and New Hampshire, we're dedicated to delivering exceptional care and compassion to our patients and their families. We're seeking a compassionate and experienced Registered Nurse - Hospice to join our team as a full-time, day shift RN.
About the Role:
As a Registered Nurse - Hospice, you'll play a vital role in providing high-quality, patient-centered care to individuals with terminal or chronic illnesses. You'll work closely with our interdisciplinary team to develop and implement personalized care plans, ensuring our patients receive the best possible care and support.
Key Responsibilities:
• Develop and implement comprehensive care plans in collaboration with patients, families, and healthcare providers • Conduct ongoing assessments of patients' clinical, functional, and emotional status to identify areas for improvement • Coordinate care plans through interdisciplinary communication and case conferences as needed • Provide education and support to patients and caregivers on care techniques, symptom management, and end-of-life care • Document patient evaluations, response, and progress towards goals in a timely and accurate manner • Communicate significant changes to patients' healthcare providers and team members
Requirements:
• Current licensure as a Registered Nurse in Vermont and/or New Hampshire (or compact license) • Graduate of an accredited school of nursing • Medical/Surgical/Oncology nursing and/or hospice care experience preferred
