We are seeking a compassionate and skilled Licensed Vocational Nurse/Licensed Practical Nurse to join our team in our Long-Term Care/Snife facility. As an LVN/LPN, you will play a vital role in delivering high-quality patient care and making a positive impact on the lives of our residents.

Key Responsibilities:

• Provide direct patient care, including administering medications, performing wound care, and monitoring vital signs • Develop and implement individualized care plans to meet the unique needs of each resident • Collaborate with the interdisciplinary care team to ensure comprehensive care and optimal patient outcomes • Educate patients and families on health promotion, disease prevention, and management of chronic conditions • Participate in quality improvement initiatives to enhance patient care and facility operations • Maintain accurate and up-to-date patient records and reports • Comply with regulatory requirements and facility policies and procedures

Requirements:

• Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of [State] • Minimum 1-2 years of experience in long-term care or snife settings • Strong communication and interpersonal skills • Ability to work effectively in a fast-paced environment • Basic life support (BLS) certification • Ability to pass a background check and obtain necessary certifications (e.g., CPR, HIPAA)
