We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to join our team in a Long-Term Care/Snafu (LTC/SNF) setting. As an LVN/LPN in this role, you will have the opportunity to provide high-quality patient care and make a positive impact on the lives of our residents.

Key Responsibilities:

• Assess, plan, implement, and evaluate patient care plans to ensure optimal health outcomes • Administer medications, treatments, and therapies as prescribed by physicians • Conduct vital signs, take medical histories, and perform other basic patient care tasks • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to develop and implement comprehensive care plans • Educate patients and their families on health promotion, disease prevention, and management of chronic conditions • Maintain accurate and up-to-date patient records and reports

Requirements:
