Job Title: LVN / LPN - LTC/SNF

We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to join our team in Long-Term Care (LTC) and Senior Living Facilities (SNF).
Key Responsibilities:
• Conduct comprehensive patient assessments to identify individual needs and develop personalized care plans • Administer medications, treatments, and therapies as prescribed by physicians • Provide wound care, dressing changes, and other skilled nursing services • Assist with daily living activities, such as bathing, dressing, and grooming • Monitor vital signs, report changes to physicians, and maintain accurate patient records • Collaborate with interdisciplinary teams, including physicians, therapists, and caregivers, to coordinate patient care • Educate patients and families on health promotion, disease prevention, and management of chronic conditions • Participate in quality improvement initiatives and contribute to a culture of excellence
Requirements:
• Current licensure as an LVN or LPN in the state of employment • High school diploma or equivalent required; associate's or bachelor's degree in nursing preferred • Minimum 1-2 years of experience in LTC or SNF settings • Strong communication and interpersonal skills • Ability to work effectively in a fast-paced environment with multiple priorities • Basic life support (BLS) certification required; advanced life support (ALS) certification preferred
