We are seeking a skilled and compassionate Physical Therapist to join our Home Health team. As a Physical Therapist with our organization, you will have the opportunity to provide high-quality patient care and make a positive impact on the lives of our patients.
Key Responsibilities:
• Provide physical therapy services to patients in their homes, as prescribed by physicians • Conduct assessments, develop and implement treatment plans, and monitor patient progress • Develop and maintain a strong therapeutic relationship with patients and their families • Collaborate with multidisciplinary teams, including physicians, nurses, and other healthcare professionals • Educate patients and their families on proper body mechanics, exercise techniques, and other related topics
Requirements:
• Current licensure as a Physical Therapist in the state where the position is located • Experience working in home health or a related setting • Ability to work independently and as part of a team • Strong communication and interpersonal skills • Basic Life Support (BLS) certification
Work Schedule:
• 4x10-hour shifts, Wednesday, Thursday, Friday, and Saturday • Minimum of 7.5 units per shift • Willingness to float within scope to meet facility needs
