Licensed Vocational Nurse (LVN/LPN) - Long-Term Care/Snife (LTC/SNF) Facility

We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN/LPN) to join our team in a Long-Term Care/Snife (LTC/SNF) facility. As an LVN/LPN, you will play a vital role in delivering high-quality patient care, supporting the well-being of our residents, and contributing to a positive and inclusive work environment.

Key Responsibilities:

• Assess, plan, and implement individualized patient care plans, ensuring the highest level of quality and safety • Provide direct patient care, including administering medications, performing vital signs, and providing wound care • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to develop and implement comprehensive care plans • Educate patients, families, and caregivers on disease management, treatment options, and self-care techniques • Maintain accurate and up-to-date patient records, including documentation of care plans, treatments, and progress • Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices

Requirements:

• Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of [insert state] • Minimum 1-2 years of experience in a long-term care or snife facility, preferably in a similar role • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams • Ability to work in a fast-paced environment, prioritizing tasks and managing multiple responsibilities • Basic life support (BLS) certification and willingness to obtain advanced life support (ALS) certification as required • High school diploma or equivalent required; associate's or bachelor's degree in nursing preferred
