We are seeking a compassionate and skilled Occupational Therapist to join our dynamic team. As an Occupational Therapist with our organization, you will have the opportunity to make a positive impact on the lives of our patients, helping them to achieve their goals and live independently.
Key Responsibilities:
• Conduct comprehensive assessments to identify patient needs and develop individualized treatment plans • Implement evidence-based interventions to enhance patient function, mobility, and daily living skills • Collaborate with interdisciplinary teams to ensure comprehensive care and coordination • Develop, implement, and evaluate programs to promote patient engagement and empowerment • Educate patients, families, and caregivers on occupational therapy techniques and strategies • Document patient progress and outcomes in accordance with regulatory requirements
Requirements:
• Master's degree in Occupational Therapy from an accredited program • Current state license to practice occupational therapy • 0-2 years of experience in a long-term care or skilled nursing facility setting (new graduates welcome to apply) • COVID-19 vaccination required, with proof of negative test results 72 hours prior to start date

Credentialing Requirements:

• State license verification • Resume • References (3) • County criminal background check (7 years for all counties lived and worked) • Federal criminal background check (7 years or more) • Multi-state criminal background check (7 years or more) • Nationwide sex offender search (7 years or more) • HHS/OIG search • GSA search • Social Security number trace • 2-step PPD or Quantiferon test • Proof of positive PPD and chest X-ray • SAM search • Washington state background check with fingerprints (facility run)
