Physical Therapist Job Description
We are seeking a compassionate and skilled Physical Therapist to join our team in Medford, OR and surrounding areas. As a Home Health Physical Therapist, you will have the opportunity to make a meaningful difference in the lives of our patients, providing them with the care and support they need to recover and thrive.
Key Responsibilities:
• Conduct patient assessments, develop and implement treatment plans, and provide hands-on therapy services to patients in their homes • Collaborate with multidisciplinary teams, including physicians, nurses, and social workers, to coordinate patient care • Monitor patient progress, adjust treatment plans as needed, and communicate with patients and families to ensure optimal outcomes • Educate patients and families on proper use of equipment, exercises, and techniques to promote independence and mobility • Document patient information and progress in EPIC/Oasis electronic medical records system
Requirements:
• 1 year of skilled home health experience (first-timers accepted) • Current licensure as a Physical Therapist in the state of Oregon • Basic Life Support (BCLS) certification • Ability to work independently and as part of a team • Strong communication and interpersonal skills
