Job Summary:

We are seeking an experienced Occupational Therapist to provide high-quality patient care in a home health setting. The successful candidate will be responsible for assessing, planning, implementing, and evaluating occupational therapy interventions to help patients achieve their goals and improve their functional abilities.

Key Responsibilities:

• Conduct comprehensive assessments of patients to identify their strengths, needs, and goals • Develop and implement individualized occupational therapy plans to address patient needs and goals • Provide occupational therapy interventions, including activities, exercises, and adaptations, to help patients improve their functional abilities • Collaborate with multidisciplinary teams, including physicians, nurses, and other therapists, to ensure comprehensive care • Document patient progress and outcomes in electronic medical records • Communicate effectively with patients, families, and healthcare providers to ensure seamless care

Requirements:

• 1 year of experience in home health occupational therapy • Current licensure as an Occupational Therapist (OTR/L) in the state of Oregon • Basic Life Support (BCLS) certification • Ability to work independently and as part of a team • Strong communication and interpersonal skills • Ability to maintain accurate and detailed records

What You'll Need to Succeed:

• Reliable transportation (your own vehicle) • Ability to work within a maximum radius of 50+ miles (occasionally) • Experience with EPIC/Oasis electronic medical records system (Oasis experience a plus)
