Job Summary: We're seeking a motivated and experienced Physical Therapist to provide rehabilitation services to our patients. As a Physical Therapist, you will be responsible for assessing, planning, and implementing treatment programs to help patients achieve their rehabilitation goals. Responsibilities:

• Conduct thorough patient assessments to identify rehabilitation needs and goals • Develop and implement individualized treatment plans to promote patient recovery and independence • Provide evidence-based physical therapy interventions, including exercises, modalities, and education • Collaborate with interdisciplinary teams, including physicians, occupational therapists, and nurses, to ensure comprehensive care • Document patient progress and outcomes in electronic medical records • Maintain accurate and up-to-date patient records and reports

Requirements:

• Current licensure as a Physical Therapist in the state of [insert state] • 1-2 years of experience in inpatient or outpatient rehabilitation settings (new grads and CFYs considered) • SNF and Rehab Optima experience preferred • Strong communication and interpersonal skills • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities • Basic life support certification (BLS) and CPR certification required
