Schedule: Rotation only 3 on 2 off 2 on 3 off
Job Type: Full-time, Part-time
About the Role:
We are seeking a skilled Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) to join our team in a Long-Term Care/Snafi (LTC/SNF) setting. As an LVN/LPN in this role, you will provide high-quality patient care, support the medical team, and contribute to a positive and inclusive environment.
Key Responsibilities:
- Provide direct patient care, including administering medications, performing vital signs, and documenting patient information
- Develop and implement individualized care plans to meet patients' unique needs and goals
- Collaborate with the medical team to coordinate patient care and ensure seamless transitions
- Educate patients and their families on health promotion, disease prevention, and management of chronic conditions
- Maintain accurate and up-to-date patient records, including medical history, treatment plans, and progress notes
- Participate in quality improvement initiatives to enhance patient outcomes and care delivery
Required Qualifications:
- Current licensure as an LVN or LPN in the state of Alabama
- Basic Life Support (BLS) certification
- Ability to work in a fast-paced environment and prioritize tasks effectively
- Strong communication and interpersonal skills
