About the Role:
We are seeking a compassionate and experienced Licensed Vocational Nurse/Licensed Practical Nurse to join our team in a Long Term Care/Senior Living facility. As a key member of our care team, you will provide high-quality patient care, support, and education to our residents.
Responsibilities:
• Assess, plan, implement, and evaluate patient care according to individual needs and goals • Administer medications, treatments, and procedures as prescribed by physicians • Conduct vital signs, take medical histories, and perform other basic patient care tasks • Develop and maintain positive relationships with residents, families, and healthcare teams • Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes • Document patient information accurately and thoroughly • Maintain a safe and clean environment, adhering to facility policies and procedures
Requirements:
• Current licensure as a Licensed Vocational Nurse/Licensed Practical Nurse in the state of NH • Minimum 1 year of experience in Long Term Care/Senior Living or related field • Strong communication and interpersonal skills • Ability to work effectively in a fast-paced environment • Basic life support certification (BLS) and ability to obtain ACLS/CPR if required
