We are seeking a compassionate and skilled Registered Nurse to join our team in a Long-Term Care (LTC) and Skilled Nursing Facility (SNF) setting.
Key Responsibilities:
• Provide high-quality patient care, including assessments, planning, and implementation of individualized care plans • Collaborate with interdisciplinary teams to coordinate patient care and achieve optimal outcomes • Monitor and report changes in patient condition, including vital signs, medications, and treatments • Educate patients and families on health promotion, disease prevention, and management of chronic conditions • Develop and maintain positive relationships with patients, families, and healthcare professionals
Required Qualifications:
• Current licensure as a Registered Nurse in the state of employment • Experience working in LTC/SNF settings, preferably in a similar role • Strong assessment and critical thinking skills • Excellent communication and interpersonal skills • Ability to work effectively in a fast-paced, dynamic environment
