Job Type: Full-time
We are seeking a compassionate and skilled Registered Nurse to join our team in a Long-Term Care/Snife (LTC/SNF) setting.
Key Responsibilities:
• Provide high-quality patient care to residents, including administering medications, performing vital signs, and managing chronic conditions • Develop and implement individualized care plans to meet the unique needs of each patient • Collaborate with interdisciplinary teams to coordinate patient care and ensure seamless transitions • Educate patients and families on health promotion, disease prevention, and management of chronic conditions • Maintain accurate and up-to-date patient records and reports
Required Qualifications:
• Current RN licensure in the state of NH • Minimum 1 year of experience in LTC/SNF or similar setting • Strong knowledge of gerontological nursing principles and practices • Excellent communication and interpersonal skills
