We are seeking a compassionate and skilled Physical Therapist to join our team. As a Physical Therapist, you will play a vital role in helping patients achieve optimal physical function and independence.

Key Responsibilities:

• Conduct thorough patient evaluations to identify areas of need and develop personalized treatment plans • Provide evidence-based physical therapy interventions to promote healing, mobility, and strength • Collaborate with multidisciplinary teams to ensure comprehensive patient care • Educate patients and families on proper exercise techniques, body mechanics, and home safety strategies • Document patient progress and outcomes in accordance with regulatory requirements

Requirements:

• Current licensure as a Physical Therapist in the state of IN • New graduates welcome to apply • SNF experience preferred • Covid-19 testing and vaccination requirements met • Must be able to pass required background checks and credentialing processes
