Registered Nurse - Home Health
Location: Elkins, WV
Schedule:
• Days, 5x8s; M-F 8a-5p
• On call (approx. 1-2 weekdays/week, 1 weekend per month)
Key Responsibilities:
• Provide high-quality patient care in a home health setting
• Develop, implement, and evaluate individualized care plans for patients
• Assess patient needs, identify risks, and implement interventions to promote health and well-being
• Collaborate with interdisciplinary teams to coordinate patient care
• Educate patients and families on health promotion, disease prevention, and management of chronic conditions
Requirements:
• Current RN licensure in the state of WV
• 1+ years of experience in home health or a related field
• BLS certification
• Travel experience preferred
Note: Travelers within 75-miles will be billed at the local rate (less $5). Local travelers are also limited to (2) 13-week contracts.
