Registered Nurse LTC/SNF

**Registered Nurse - LTC/SNF** **Location:** Lebanon, NH **Schedule:** Every other weekend rotation, with shifts on weekdays and weekends. **Job Type:** Full-time **Required Qualifications:** • Current registration as a Registered Nurse (RN) in the state of NH • Experience working in a long-term care or skilled nursing facility (LTC/SNF) setting • Strong clinical skills and ability to assess, plan, implement, and evaluate patient care • Excellent communication and interpersonal skills • Ability to work effectively in a fast-paced environment **Key Responsibilities:** • Provide high-quality patient care, including assessment, planning, and implementation of care plans • Develop and maintain relationships with patients, families, and healthcare team members • Collaborate with interdisciplinary teams to coordinate patient care • Monitor patient progress and adjust care plans as needed • Maintain accurate and up-to-date patient records • Adhere to HIPAA guidelines and maintain confidentiality of patient information
