LVN / LPN - LTC/SNF Location: Camden, ME Schedule: Nights Job Type: Full-time/Part-time We are seeking a licensed LVN/LPN to join our team in a Long-Term Care/Senior Living facility. As a key member of our healthcare team, you will provide high-quality patient care and support to our residents. Responsibilities: • Assess, plan, coordinate, and evaluate patient care • Develop, implement, and revise individualized care plans • Administer medications and treatments as prescribed • Monitor vital signs and report changes to the healthcare team • Educate patients and families on health and wellness • Collaborate with interdisciplinary teams to coordinate patient care • Document patient information accurately and thoroughly Requirements: • Current licensure as an LVN/LPN in the state of Maine • Experience working in a Long-Term Care/Senior Living facility • Strong communication and interpersonal skills • Ability to work effectively in a fast-paced environment • Basic life support certification (BLS) required
