**Therapist - Social Worker** **Location:** Edmonds, WA 98026 **Shift:** Days We are seeking a compassionate and experienced Therapist - Social Worker to join our team. As a Therapist - Social Worker, you will provide high-quality care to our patients, utilizing your expertise in social work to coordinate care, manage cases, and advocate for our clients' needs. **Key Responsibilities:** • Provide case management and discharge planning services to patients, ensuring seamless transitions and optimal outcomes • Conduct initial evaluations, develop plans of care, and coordinate care with various disciplines and services • Utilize InterQual Criteria and Milliman Guidelines to ensure medical necessity and appropriate care • Document patient information, care delivery, and progress towards goals • Collaborate with patients, families, caregivers, and healthcare professionals to achieve desired outcomes • Develop and implement care coordination activities, including referrals to rehab and LTC facilities • Stay up-to-date on regulatory requirements, including CMS, CPT coding, and HIPAA guidelines **Requirements:** • Master's degree in Social Work (MSW) from an accredited program • Active Washington state license to practice as a Therapist - Social Worker • 2+ years of experience in social work, with a focus on case management and care coordination • Strong knowledge of cultural issues, NASW Code of Ethics, and Standards of Home Health Care • Ability to work effectively in a fast-paced environment, with attention to detail and excellent communication skills • Familiarity with Epic and Cerner electronic medical records systems **Preferred Qualifications:** • Associate clinical license or LCISW • Experience with utilization management and concurrent review • Knowledge of Geriatric Depression Scale and Mini-Mental State Examination • Experience with Discharge Summary and patient/caregiver teaching/learning major disease processes If you are a motivated and compassionate social worker looking for a new challenge, please submit your application.
