LVN / LPN LTC/SNF

Job Title: LVN / LPN - LTC/SNF Location: Middlebury, VT Schedule: Monday to Sunday Job Type: Full-time Travel Assignment Required Qualifications: • Dementia capable care certification preferred • A minimum of 1 year of experience • Prior travel required • EPIC experience from within the past 2 years is required and nonnegotiable Key Responsibilities: • Provide high-quality patient care in a long-term care setting • Collaborate with interdisciplinary teams to develop and implement patient care plans • Administer medications and treatments as prescribed by physicians • Conduct vital signs, take medical histories, and perform other patient assessments • Educate patients and families on health promotion, disease prevention, and management of chronic conditions Key Considerations: • Must be able to work 2 out of the 4 holidays: Thanksgiving, Christmas, Christmas Eve, and New Years Day • Must be able to lift, transfer, and position patients as needed • Must be able to work in a fast-paced environment with frequent interruptions • Must be able to maintain confidentiality and handle sensitive patient information Certifications and Training: • Dementia capable care certification preferred • EPIC experience from within the past 2 years is required and nonnegotiable Patient Population: • Patients with dementia and Alzheimer's disease • Patients with chronic medical conditions, such as diabetes and heart disease • Patients with cognitive impairments and behavioral changes Unit and Facility Information: • Long-term care unit with 4 beds per room • Skilled nursing facility with a variety of medical equipment and technology • Collaborative team environment with opportunities for professional growth and development
