LVN / LPN - LTC/SNF Keene, NH Full-time, Days Key Responsibilities: • Provide high-quality patient care in a long-term care and skilled nursing facility setting • Assess, plan, implement, and evaluate patient care plans to meet individual needs • Administer medications, treatments, and procedures as prescribed by physicians • Collaborate with interdisciplinary teams to coordinate patient care Required Qualifications: • Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) • Current certification in basic life support (BLS) and cardiac arrest response • Ability to maintain accurate and detailed patient records Preferred Experience / Skills: • Experience working in long-term care and skilled nursing facilities • Knowledge of medical terminology and anatomy • Ability to communicate effectively with patients, families, and healthcare teams
