Registered Nurse - LTC/SNF

Key Responsibilities:

• Provide high-quality, patient-centered care to residents in a long-term care or skilled nursing facility setting • Assess, plan, implement, and evaluate patient care plans to meet individual needs and promote optimal health outcomes • Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions • Educate patients and families on health promotion, disease prevention, and management of chronic conditions • Maintain accurate and timely documentation of patient information and progress

Required Qualifications:

• Current RN licensure in the state of NM • 1+ year of experience in LTC/SNF or related field • Basic Life Support (BLS) certification • Ability to lift, transfer, and position patients as needed

Preferred Experience / Skills:

• Experience working in LTC/SNF or acute care settings • Knowledge of medical-surgical nursing principles and practices • Familiarity with electronic medical records and documentation systems
