Key Responsibilities: • Provide high-quality patient care in a long-term care setting, including administering medications, performing vital signs, and assisting with daily living activities. • Develop and implement individualized care plans to meet the unique needs of residents, in collaboration with interdisciplinary teams. • Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements. • Educate patients and families on health promotion, disease prevention, and management strategies.
Required Qualifications: • Current licensure as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) in the state of NM. • Current certification in Basic Life Support (BLS) or higher. • Ability to lift, transfer, and position patients as needed.
Preferred Experience / Skills: • Experience working in long-term care or skilled nursing facilities. • Knowledge of gerontological nursing principles and practices. • Bilingual or multilingual skills, with the ability to communicate effectively with diverse patient populations.
