Key Responsibilities: • Provide physical therapy services to patients in a home health setting • Conduct assessments, develop treatment plans, and implement exercises to improve patient mobility and function • Collaborate with healthcare teams to coordinate care and ensure seamless transitions
Required Qualifications: • Physical therapist licensure in the state of Missouri • Basic Life Support (BLS) certification • Home Health experience
Preferred Experience / Skills: • Experience working in a home health setting • Familiarity with Epic charting system
